BronxWorks' Street to Home program has been selected as a merit finalist in the Mutual of America’s Community Partnership Award program.  It is a national competition that receives hundreds of applications each year.  Since the award program began 17 years ago, BronxWorks is the only Bronx-based organization among the winners or finalists.  

Street to Home began 18 years ago, when BronxWorks – founded in 1972 with the goal to lessen the burden of poverty for individuals and families living in the South Bronx – teamed with Montefiore Medical Center (MMC) and Care for the Homeless (CFH) to tackle chronic homelessness in the Bronx. The program combines BronxWorks' social work expertise with MMC's and CFH's medical and behavioral resources and is funded by New York City’s Department of Homeless Services.

BronxWorks employs a continuum of care strategy which is implemented by BronxWorks’ department of adult homeless services. The strategy offers medical and mental health care and housing options for single street homeless adults in the borough.  This includes a borough wide mobile street outreach team, a 24-hour drop-in center, and a 50-bed safe haven program, all of which operate with the objective to permanently house homeless individuals. These programs work together seamlessly and allow BronxWorks to work with street homeless individuals from the time they are on the street to the time they are placed in permanent housing. BronxWorks staff has an excellent understanding of their clients and their issues, and can refer them to the housing arrangement that is the best fit for their needs.  

Dr. Andrea Littleton, a physician at MMC and BronxWorks first medical director, accompanies the mobile outreach unit twice a month, providing on-the-spot medical care and prescriptions to the chronically homeless.
